Power of Attorney for Health Care - This document permits a loved one to make health care decisions for you in the event you cannot.   If you do not have this document Court intervention will be required.  This even applies where the patient is married.  That is, spouses cannot make health care decisions for each other.

HIPPA Release - This document releases your health care information to your loved ones.   Your health care information may not be released to even a spouse without a release or Court intervention.
